5 things Emily's handoff cost us We fixed them without replacing anyone - watched it happen in Seattle, Tuesday 9:17 AM (Emily’s cracked travel mug on the table, tiny detail that stuck with me). 1) Ownership drift - tasks moved but nobody owned outcomes - so momentum died. 2) Silent assumptions - everyone assumed someone else followed up - so nothing happened. 3) Missing acceptance criteria - work was “done” but not usable by the next person. 4) Tiny tool gaps - a missing tag in the CRM lost visibility - a five-second checkbox. 5) Bad defaults - email threads were the default handoff (painful for scaling). 6) Feedback blindspots - no simple loop for quick corrections - small errors compounded. 7) Incentive mismatch - the person who finished a task didn’t benefit from its downstream success. What we did instead - a short checklist + one-line owner on every card + a tiny CRM tag; and we taught people to hand off in 60 seconds (no theatrical explanations, just the facts). (Yes, it felt boring at first - boring wins.) The real point - this wasn’t a people problem - it was a handoff problem.
